What We’re Looking For Trillium Health Resources has a career opening for a Population Health Nurse Supervisor to join our team!
The Population Health Nurse Supervisor is responsible for the administrative oversight and clinical supervision of the Population Health Nurses who provide care management and care coordination services, linkage/referrals, and discharge planning activities. Reporting to the Population Health Nurse Manager, the Population Health Nurse Supervisor is available to licensed and non-licensed staff to perform clinical triage, lead on emergency or crisis calls/contacts, or provide guidance and support, as needed. On a typical day, you might: Monitor and oversee the activities of the Nurse Team to ensure the needs of members, families, and stakeholders are addressed appropriately, expeditiously, and in a courteous manner.
Identify member needs and escalate appropriately to the Nurse Manager, Director, or other appropriate Trillium staff. Monitor and review data to ensure performance measures and benchmarks are consistently met. Provide education to staff to ensure performance improvement and to maintain adherence and compliance with accreditation standards.

Licensed in North Carolina by the State Board of Nursing as a Registered Nurse (RN). Must have a valid driver’s license.
Must reside within North Carolina. Must be able to travel within catchment as required. Preferred: Experience working with acute care discharge planning and/or utilization management, including behavioral health, mental health, and physical health care needs.
Experience coordinating physical healthcare needs. A minimum of one (1) year of experience providing direct supervision of staff (2 or more employees) that provide BH/MH/SU and/or IDD services. Deadline for Application: Monday, August 10, 2026 @ 11:59pm
